After the consultation, we’ll guide you to schedule your in-office exam and scans if it feels like a good match.

We know many parents are scheduling on behalf of their child — that’s great! Just make sure:
- You enter your child’s name and information (not your own) in the form below
- You select and fill out the correct Pediatric Paperwork for their age after booking
If you’re scheduling for yourself, go ahead and use your own info.
If you’re scheduling for your child, enter their info — even if you’re the one filling it out.
